MAP and St. Michael’s Hospital Navigator Homeless Outreach Services work with patients experiencing homelessness to improve their care, connect them to follow up services, and help them get housed.
There are more than 10,000 people experiencing homelessness in Toronto. People experiencing homelessness often also experience serious health conditions and frequently visit the Emergency Department or are admitted to hospital. MAP and St. Michael’s Hospital run a number of Navigator Homeless Outreach Services designed to improve health, assist with housing and lessen the burden on the hospital system. These services provide case management and navigation services in the community for one to six months. Following MAP’s goal to develop and scale programming to increase health equity, we request $500,000 to scale and integrate these services into one model in order to better serve people experiencing homelessness and create a more coordinated system.
