N Street Village will pilot enhanced mental health services for older women at risk of chronic homelessness to help them successfully age-in-place in supportive housing.
Current Permanent Supportive Housing models do not address the unique needs of the aging homeless population, which include disproportionate prevalence of substance use disorders and mental health conditions. N Street Village will design a new approach, interweaving geriatrics-focused mental health care across PSH programs and developing novel supports and pathways to support services that address the issues of women aging-in-place and reduce returns to homelessness. The Village will forge partnerships to learn and implement best practices, then to share those identified through evaluation for scaling and replication.
