This replicable model project will exponentially boost HELP’s ability to process and act on requests for assistance avoiding homelessness in the Bronx in New York City by adding: 1) a client management system for triaging cases and queueing clients as they arrive at Homebase and Supportive Services for Veteran Families (SSVF) homelessness prevention program sites to ensure highest-risk cases are prioritized and smooth service flow; 2) a web-based application to enable submission of assistance applications and required documentation electronically—increasing the efficiency and empowering remote applications; and 3) a digital interface to bridge the multiple electronic data tracking systems for streamlined data entry and easier, standardized reporting to ensure data-driven decision-making
